Office policies - our policies regarding payments, missed appointments, etc.

Medical records release - we always like to have sent to us copies of all your laboratory reports from the past 12 months and any other information collected by other health care providers that pertain to the condition you are seeking our help with. This form is completed by you and submitted to the health care provider(s) who's records are to be sent to us.
